| Registration Procedures |

| Participants are requested to fill out the Registration Form on the official web site (http://www.plando.co.jp/icrs2004/) or attached Registration Form (Form A). |

| Payment |
|
|
| Payment for the registration fees should be made in Japanese yen(indicated as JPY)by bank transfer, bank draft, or credit card(American Express, MasterCard or VISA). Please note that personal checks cannot be accepted. The remittance should be made to the following bank. |

| Confirmation |

| Upon receipt of the registration form, the Secretariat will send a Confirmation Card by e-mail or FAX containing your registration number. Please bring this card to the registration desk at your arrival. |
|
| Cancellation |
|
|
| The registration fee will be refunded only for cancellations received in writing and faxed to the Secretariat on or before April 30, 2004. Thirty percent(30%)of the fee will be deducted for cancellation. Please indicate clearly the bank, branch, and account name and number to which the refund should be made.
If you cannot attend, you may send a substitute person. The original registrant must submit a written authorization for such a change.

| Passport and Visas |
|
|
| All travelers entering Japan must have a valid passport. In addition, participants coming from certain countries must have a visa issued by the Japanese consul in their country. Please obtain advice for details from your nearest Japanese embassy or consulate.
Participants requiring a visa for entry into Japan are requested to fill out the attached Questionnaire for Visa Form(Form D)and send it to the Secretariat by e-mail or FAX.
Once the completed questionnaire and the registration fee have been received, the Secretariat will send applicants the support documents needed to apply for a visa. |
